Accident forms completed by parents

Recently we have found that accident forms completed by parents do not state the parents name when opened but the name of the practitioner who has opened it to read. I have attempted to over ride the practitioners name but today I was unable to so wrote a comment in the notes section. On the accident list it will sometimes show the parents name but at other times it is the practitioners name eg AH 16.06.24 dad sent in an accident form but it now says that it was completed by JL and I am unable to change this. I have spoken to a member of staff and they have suggested I fill out this form. Many thanks
