Child's Voice on Accident Sheet/Scotland - Care Inspectorate

Accident incident sheet would benefit from a box that we can put the child's voice into following the accident/incident - how did they feel, what could have been done to keep them safe etc.

Also in accident/incident it states ofsted, in Scotland we are registered with the Care Inspectorate.
